Register a new patient
Let's create a new chart — with demographics and insurance in place, the front desk can book and bill right away.
You'll set up a patient's chart in CareChart so your front desk can schedule appointments and process insurance claims. A complete chart with verified insurance eligibility means faster bookings and fewer billing delays.
In Patients, click New patient to get started.
Enter the patient's legal name exactly as it appears on their insurance card.
Add the date of birth — CareChart checks for possible duplicate charts as you type, so you won't create a second one by mistake.
CareChart flags potential duplicates as you enter the date of birth, helping you avoid creating multiple charts for the same patient.
Select the insurance plan and member ID — eligibility is verified in real time, no phone call needed.
Real-time eligibility verification confirms the patient's coverage is active, so you know immediately if claims will process.
Click Create chart — an MRN is assigned and the patient can be booked right away. Nicely done!
Once the chart is created, an MRN (medical record number) is automatically assigned and the patient appears in your booking system right away.

With the patient's chart created and insurance verified, your front desk can now book their first appointment and begin collecting any additional intake information needed for their visit.
